1095-B Tax Form
New Tax Form Coming Your Way
One provision of the Affordable Care Act requires the Fund Office to provide you
with a statement for your taxes called a 1095B. This is a form that goes to plan
participants that have had medical coverage provided by Local 399’s H&W Plan in 2015.
This is the form that has prompted the Fund Office to actively pursue social security number
for your eligible dependents. The form ties in to the Act’s requirement that everyone have
health insurance or pay a tax penalty on their return next year.
