ATTENTION LOCAL 399 H&W PARTICIPATING MEMBERS:

Effective 01/01/2025, we will be moving back to CVS Caremark
for our pharmacy benefits.

You will be receiving multiple communications about this
change in your mailbox beginning in 
mid-December including
welcome information, instructions for using the home delivery service,

contacting customer service, and registering to use
the online portal and app.

 

Both CVS Caremark and BlueCross BlueShield
will mail you a new 
I.D. card before January 1, 2025.
Both will have the updated pharmacy 
data located on it for you
to use to obtain your prescriptions. Keep it 
handy and begin
using it January 1, 2025. Be sure to discard your previous

BlueCross BlueShield and OptumRx cards after January 1st
to 
avoid billing and payment errors.

 

Plan benefits are not changing; however, CVS Caremark’s formulary
(list of covered medications) may differ from 
what was in place.
The formulary is posted on our Health 
& Welfare webpage.
You will receive a letter in the mail 
from CVS Caremark
if a medication you are currently on is not part of the
new formulary along with similar alternative medications.
